Project Background:
India is one of the highest tuberculosis (TB) burden country, accounting for an estimated onefourth (estimated TB incidence in India -2950000) 1 of the global incident cases. Drug-resistant tuberculosis (DR-TB) presents a major public health problem in India as the country contributes over 27% of global DR-TB cases.2 It is estimated that around 56% of MDR-TB cases remain undiagnosed in India3 . The remining experience considerable delays in diagnosis and treatment initiation leading to sub-optimal treatment outcomes. In recent decades, India has been proactive in its battle against TB, even implementing a revised National Strategic Plan (NSP) to eliminate TB by 2025. However, to achieve this ambitious goal, the country will need to take a multifaceted approach with respect to its management of DR-TB. Cognizant of the alarming need for strengthening DRTB diagnostic capacity across the country, the NSP (2017-2025) also emphasis on the scale up UDST coverage as key priority for the Nation TB elimination Programme (NTEP)

Against this background, FIND will be implementing a project to effectively understand and address the gaps across the patient care cascade including access to quality diagnosis and subsequent linkages to treatment service to obtain a granular understanding of the key bottlenecks and barriers in existing Universal Drug Susceptibility Testing (U-DST) coverage across both the public and private sectors in a high TB burden district in UP.

Project Goal:
Strengthen U-DST coverage for increasing DR-TB diagnosis and reducing the gap of missing DR-TB patients in identified high TB burden geographies to reduce transmission and for better treatment outcomes.

Project objectives:
  • Identify and quantify key bottlenecks and barriers in the DR-TB diagnosis cascade across public and private sectors.
  • Develop, implement, and demonstrate sustainable and replicable pilot strategies for strengthening DR-TB diagnosis and treatment linkages across public and private sector.
  • Prepare a guidance document for conducting DR-TB diagnosis cascade analysis at district level and practices and processes for addressing critical gaps and barriers.
