Role objectives |
Responsibilities and Accountabilities Core Functions / Responsibilities: -Conduct triage and initial patient assessment prior to be examined by medical doctors; -Prior to PE they should undergo history review and vital sign measurement with the nurse. The nurse should: a)Enquire about ·Any changes to the client’s medical condition since the last evaluation ·Presence of signs or symptoms that may indicate TB or other infectious disease (cough, fever, weight loss, skin rash, diarrhea etc.)
·Possibility of pregnancy in women of
child-bearing age. b)Measure vital signs: ·Temperature; ·Respiratory rate; ·Blood Pressure, for persons with one or more of oAge ≥ 40 years; oPregnancy; oHistory of hypertension or other cardio-vascular disease; oHistory of renal disease. c)Evaluate for malnutrition: ·Measure height and weight and calculate WHZ, BMIZ, or BMI, or ·For pregnant women, measure MUAC. d)Measure O2 saturation, for persons with ·Cardiac or respiratory SMC’s; ·Anemia; ·Clinical shortness of breath. e)Obtain a specimen for hemoglobin measurement for persons with ·SMC’s (particularly cardio-respiratory) that pose potential flight risk; ·Pregnancy; ·Moderate to severe malnutrition identified at initial examination; ·Anemia identified at initial examination; ·Reduced O2 saturation. f)Measure blood
glucose level for persons with diabetes - Help in development and organization of the registration system and documentation for beneficiaries, - Maintain inventory of the clinic’s equipment, medicines and consumables, - Ensure beneficiaries’ confidentiality, - Attend scheduled programme meetings, - Assist other project staff whenever needed, Any other responsibility assigned by his/her supervisor. |
