Objective/purpose of the consultancy/services

 To facilitate the preparation process and organization of the two trainings by Director and Second trainer which are aimed at the building capacity of leading experts in the provision of essential health services to young children in a critical health conditions in order to increase their chances to survive.

Background

In the frame of National Health Care Reform Programme Den Sooluk (2012-2017) and Perinatal Care Improvement Program in the Kyrgyz Republic for 2008 – 2017, the reduction of maternal and child mortality rates through increased coverage by evidence-based health interventions was highly emphasized.

The in-depth analysis has brought to surface a number of gaps that need to be addressed in a comprehensive manner. These gaps and deficiencies are found first of all in the knowledge and practical skills of health providers to timely provide quality services to sick children in a severe conditions, in infrastructure, availability of life-saving equipment, supplies and integration of different levels of health system to provide services in a continuum way. Appropriate pediatric emergency care is considered as highly important in the package of services to ensure survival of young sick children.

Up to now, according to joint plan  of the Ministry of Health and UNICEF, several trainings related to management of children in a severe health condition were rolled out (BLS, PALS).

Some of the observations noted by PALS trainers were as following:

Equipment: there was a range of equipment supplied through various sources to enable this training to take place.

Capacity of health providers: What was evident that although much of this equipment would be considered basic to resuscitation of children, not all of those trained were familiar with this equipment nor had necessarily used it previously.  This would apply to the ambu bags, to laryngoscopes and to the intubation equipment.  Even pulse oximeters were not that familiar to some staff.

As part of comprehensive program to improve the quality of services provided to children at hospitals, majority of health facilities were equipped with ventilation equipment with organization in a parallel of above mentioned trainings. But still, as it is confirmed by the MOH, the practical skills and knowledge of health providers to provide essential treatment to children are limited and not based on up to date global standards of care.  This is confirmed by the fact, that during last three decades the level of mortality of children under 2 years old at the first day of the hospitalization is still remained high, and this fact reflects the low capacity of health providers to timely identify critical situation in child health and low quality of provided services.

To address these gaps and bottlenecks in the quality of provided services to children, it was agreed with MOH to support organization of two practical trainings focused on improvement of skills of health providers to adequately use available ventilation equipment in order to improve chances of young children in a sever health conditions to survive.

These activities will be implemented in the frame of RWP of UNICEF's Health and Nutrition Section signed and agreed with the MOH  for 2016-2017.

Primary beneficiaries: Doctors and nurses involved into provision of health services to sick children in a critical health conditions.

Secondary beneficiaries: sick children in a severe health conditions.

 

Tasks & Project Deliverables

Director of course and main facilitator

7 days on-line and 10 days in the country

1. To develop content of the 5 days training on the use of ventilation equipment within the treatment process of sick young children in a most common severe health conditions related to respiratory track. The content of the training module should be agreed in details with the National experts of the MOH through on-line communication. It is planned that first two days of the training should be theoretical. Last three days should be practical. Practical sessions should be organized for doctors and middle level health professionals separately. Director of the course is expected to facilitate theoretical and practical sessions for doctors.

2.  As a Director of the course to guide the Second trainer who will be involved into facilitation of theoretical session and will be facilitating the practical session for middle level health professionals

3.  To facilitate two 5 days trainings on the use of ventilation equipment within the treatment process of sick young children in a most common severe health condition.

4.  To finalize module and presentations of the trainings, provide recommendations to the Clinical Standard on ventilation treatment of sick children, which will be developed by the MOH and prepare mission report with recommendations

Trainer of the course

3 days on-line and 10 days in the country

1.  To develop content of the 3 days practical session of the training on the use of ventilation equipment within the treatment process of sick young children in a most common severe health conditions related to respiratory track. It is planned that Second trainer will be providing support to the Director of course in the facilitation of the theoretical part and will be fully responsible for the facilitation of the practical session for middle level health professionals.

2. To facilitate two 5 days trainings on the use of ventilation equipment within the treatment process of sick young children in a most common severe health condition. (to provide support to Director of the course during theoretical session and facilitate practical sessions for middle level health professionals

Duration and concrete schedule

For Director of the course:

17 days during the period starting from 1 October till 15 December 2017.  7 days on-line work and 10 days in the country: 

  • 5 days on-line for preparation of the Training package
  • 1 day preparation for training + 5 days training in Bishkek city
  • 1 or 2 days weekends or transfer to Osh and back to Bishkek
  • 1 day preparation for training + 5 days training in Osh
  • 2 working days on-line for finalization of the module and writing the mission report with recommendations).

Qualification/level requirement

  • University degree, advanced level in Pediatrics, Child Resuscitation;
  • broad experience in organization of such type of requested trainings, based on globally recognized modules and approaches;
  • Certification of position of International or Master trainer on this type of trainings;
  • practical experience of working in child resuscitation units no less than 5 years;
  • Initiative and ability to organize independently; sense of working thoroughly; good organizational and communication skills

 Languages

  • Spoken in Russian is requirement, written and spoken English is an asset

8. Duty station and official travel involved

Bishkek (5 days of training+1 day for preparation+1 or 2 travel days or weekends) and Osh (5 days of training+1 day for preparation+1 or 2 travel days or weekends) cities, Kyrgyzstan

If UNICEF determines that the Consultant needs to travel in order to perform his or her assignment that travel shall be specified in the contract and the Consultant's travel costs shall be set out in the contract. In cases where travel costs have not been set out in the individual contract, the consultant and individual contractors are expected to submit, within ten days of completion of a specific travel, a voucher for reimbursement of travel expenses to the appropriate Operations / Human Resources manager.

Reimbursements shall only be processed if travel was duly authorized in the individual contract or by an authorized manager of the contracting division or office, in writing and prior to the travel. Consultants and individual contractors are responsible for assuming costs for obtaining visas and travel insurance.

Travel paid for by UNICEF in advance, and travel costs reimbursed after travel, shall be based on economy class travel, regardless of the length of travel, subject to exceptional approval of business class travel by the Head of Office/ Division Director, for example for medicals reasons when certified by UN Medical Services in New York. Daily subsistence allowance (DSA) for official in-country field travels shall not exceed the daily rates established for specific country regions as promulgated by the International Civil Service Commission (ICSC).

For the Trainer:

13 days during the period starting from 1 October till 15 December 2017. 3 days on-line work and 10 days in the country

  • 3 days on-line for preparation of the Training package
  • 5 days training in the Bishkek city
  • 2 not-working days and transfer to Osh and back to Bishkek
  • 5 days training in Bishkek
