Background/IRC Summary:  The IRC in New Jersey helps nearly 1,000 refugees, immigrants, and other community members each year to achieve thriving, self-sufficient lives through a continuum of programs and services including refugee resettlement, immigration, education and learning and economic empowerment.
The Intensive Case Management (ICM) program provides strengths-based, client-centered case management for refugees, asylees, special immigrant visa holders (SIVs) and other eligible populations who are experiencing barriers to self-sufficiency in the United States. Populations served may include but are not limited to: individuals with medical challenges, individuals with mental health needs, single parents, LGBTQ+-identifying individuals interested in case management support, isolated women and/or women at risk of gender-based violence, survivors of torture or trauma, and elderly individuals. 
Job Overview/Summary: The IRC in New Jersey’s Preferred Communities Intensive Case Management (ICM) program assists with the long term, successful integration of newly arrived refugees and other humanitarian entrants with medical and mental health needs. The Intensive Services provides holistic case management services, frequently focused on healthcare navigation, ensuring that clients are connected to necessary services and resources both within and outside of IRC’s programs. S/he supervises BSW and/or MSW level interns and collaborates closely with colleagues internally to best serve refugee families. S/he participates in statewide refugee wellness council meetings to build capacity to address the medical and mental health needs of newly arrived refugees both organizationally and throughout Union county and surrounding counties.
Major Responsibilities: Responsibilities include but are not limited to: 
Case Management and Referrals:
•Provide trauma-informed comprehensive case management for vulnerable refugees, asylees, and SIVs to help them feel safe, empowered, and stable.
•Complete individualized client intakes, including orientation to services, assessment of client needs, and development of client-specific safety plans.
•Develop individualized service plans for each client with an emphasis on the client’s strengths, barriers and needs. Assist clients to set and achieve goals leading to economic and social self-sufficiency.
•Connect clients to community resources including physical and mental health services, public benefits, transportation, low-income housing, social and community support, legal services, food access, childcare resources, etc.
•Identify appropriate medical providers, community partners, and other social service agencies and advocate for refugees to have full, ongoing access to medical and mental health services.
•Conduct Refugee Health Screening (RHS)-15 to assess refugees’ needs for further assessment
•Facilitate or oversee facilitation of Pathways to Wellness (PTW), in consultation with casework supervisor.
Partnerships and Community Engagement:
•Build and maintain partnerships with external community service providers to improve client access. Assist clients in accessing ongoing care.
•Represent IRC at community meetings and events, build relationships with local stakeholders and expand IRC’s visibility among community partners.
•Establish clear referral systems with community partners, both for sending and receiving referrals. 
•Participate in office wide and ICM-specific meetings, trainings, and staff development activities.
•Support coworkers in responding to the urgent or unanticipated needs of vulnerable clients.
•Provide oversight to undergraduate, graduate, and other student interns and volunteers.
Documentation and Reporting:
•Maintain excellent case file quality by ensuring complete and timely documentation of intakes, assessments, service plans and case notes. 
•Monitor and track service delivery to ensure compliance with program guidelines. Complete and submit client assessments at required intervals to track client’s progress toward service plan goals.
•Prepare, distribute and document ICM clients’ direct assistance and donations.  
•Assist with completion of reports as required by funders and/or donors.
•Comply with and remain up-to date on all policies, procedures and best practices of the agency.
•Other related duties as assigned.

Position directly supervises: BSW and MSW interns
Indirect Reporting: 
Other Internal and/or external contacts: 
•Internal: coordinate closely with youth program coordinator, resettlement caseworkers, employment specialists, adult education coordinator, extended services caseworker and immigration supervisor and coordinator
•External: Health clinics, welfare offices, social service agencies
Job Requirements: 
•Bachelor’s Degree required; preferably in public health, social work, human services or related field; Master’s degree in Social Work strongly preferred.
•At least two years case management experience required.
•Experience working with refugees, victims of human trafficking, immigrant populations or vulnerable populations strongly preferred.
•Understanding of New Jersey welfare, health insurance and medical systems preferred.
•Proven ability to balance direct client services with documentation and other compliance measures.
•Aptitude for working through intense mental health and/or medical situations with clients, often in an emergency situation.
•Demonstrated success working and communicating effectively in a multi-cultural environment.
•Proven ability to contribute both independently and as a key team member.
•Self- starter with excellent problem solving skills combined with the proven ability to multi-task, prioritize duties, and manage time effectively.
•Fluent in English, both spoken and written; bilingual ability in one of the predominant languages of the local client base is desired: Spanish, Arabic, Swahili, French, Dari or Pashto.
•Proficient in Microsoft Office applications (Word, Excel, Outlook)
Working Environment:  Standard office work environment, along with some field work, including home visits, health providers and other community-based partners. Use of personal, insured vehicle is preferred, but field work may be completed via public transportation.  Occasional evening and/or weekend work required. In COVID-19 environment, work is 100% remote as of September 2020. Re-opening plans are still under review.
