Background

The Syria crisis is often described as the worst humanitarian catastrophe since the end of the Cold War. Inside Syria, 7.6 million people are internally displaced and 12.2 million are in need of humanitarian assistance, with 4.8 million in hard-to-reach areas. There are 3.8 million Syrian refugees in neighboring countries. This is no short-term humanitarian episode. The devastating human consequences to huge numbers of people will endure for decades. The destruction of relationships, communities, livelihoods, homes and infrastructure will take years to repair.

IRC is offering a robust humanitarian response to the Syria crisis with an annual budget in excess of $140 million and a rapidly expanding portfolio, supported by more than 1,250 staff in the region. IRC is undertaking programs in Syria and the neighboring countries of Turkey, Iraq, Lebanon and Jordan in the fields of health, child protection, education, women’s protection and empowerment, NFI and food distribution, cash assistance, water and sanitation, and livelihood programming. Our work in these challenging settings gives rise to some of the most pressing issues facing contemporary humanitarian action, including questions of access, security, funding and coordination.

Job Overview/Summary:

The Community Health Worker (CHW) will work under direct supervision of Community Health Officer. She/He will be responsible for helping patients and their families to navigate and access community services mainly health related services, and other resources, and adopt healthy behavior changes. As a priority, activity will promote, maintain, and improve the health of people and their families in the area of intervention. Provide social support and informal counseling, advocate for individuals and community health needs, provide in home services such as, and blood pressure screening, , MUAC screening for children and alert the health management for any Malnutrition cases or any potential morbidity cases that need follow up such as cases set in the EWARN system. Community outreach, such as home visits and group education and health screenings will be required. Referral of patients and follow up with them in their homes to ensure correct adherence to treatment plans set by doctors.

Job Responsibilities

The CHW is expected to undertake the following tasks:

• Building a trust relationship with visited families and community.
• Creating connections between vulnerable populations and healthcare systems
• Ensuring cultural competence among healthcare professionals serving vulnerable populations

• Educating health system providers and stakeholders about community health needs

• Providing culturally appropriate health education messages on topics related to Non-communicable diseases, Communicable diseases prevention, physical activity, nutrition, mental health, GBV, child and mother health, and reproductive health.
• Supporting health needs assessment activities.
• Giving health messages in the community in important topic to the community, making the families aware of the available service provided by IRC or other health actors in the area of intervention.
• Measuring improvement of community awareness by making pre and posttest surveys with support of M&E team.
• Collecting data and convey information to policymakers to inform policy change and development
• Providing informal counseling, health screenings, and referrals
• Building capacity to address health issues
• Following up important medical cases in IRC facilities unit to screen the progress and improvement of them.

• Reporting daily, weekly and monthly as agreed with supervisor

He/she works as team member and cooperate effectively withother colleagues

Cooperative and respectful relationship with population, colleagues in the clinic and management of IRC.

لمحة عامة عن العمل / ملخص

سيقوم عامل الصحة المجتمعية بالعمل تحت الإشراف المباشر لمسؤول الصحة المجتمعية. سوف يكون عامل صحة المجتمعمسؤولاً عن مساعدة المرضى وعائلاتهم في التعرف على الخدمات المجتمعية التي تتعلق بشكل رئيسي بالخدمات المرتبطة بالصحة والموارد الأخرى ، و مساعدتهم في تبني تغييرات سلوكية والعمل بسلوكيات سليمة. على قائمة ألأوليات ، سيعمل عامل الصحة المجتمعية على تعزيز صحة الناس وعائلاتهم والحفاظ عليها وتحسينها. توفير الدعم الاجتماعي والاستشارة غير الرسمية ، وتوفير الخدمات المنزلية مثل , فحص ضغط الدم ، وفحص MUAC للأطفال وتنبيه الإدارة الصحية عن أي حالات سوء تغذية أو الامراضيات المتوقعه و التي تحتاج إلى متابعة مثل الحالات المدرجة في نظام EWARN. و ما يترتب عليها من ﺣﺎﺟﺔ إﻟﻰ ﺗﻮﻋﻴﺔ اﻟﻤﺠﺘﻤﻊ ، ﻣﺜﻞ اﻟﺰﻳﺎرات اﻟﻤﻨﺰﻟﻴﺔ واﻟﺘﻌﻠﻴﻢ اﻟﺠﻤﺎﻋﻲ واﻟﻔﺤﻮﺻﺎت اﻟﺼﺤﻴﺔ. إحالة المرضى ومتابعتها في منازلهم لضمان الالتزام الصحيح لخطط العلاج التي وضعها الأطباء.

 بناء علاقة ثقة مع العائلات والمجتمع.

 ربط الفئات المستضعفة من السكان بنظام الرعاية الصحية

 ضمان كفاءة المتخصصين في الرعاية الصحية الذين يخدمون الفئات المستضعفة من السكان

 تثقيف مقدمي الخدمات الصحية حول الاحتياجات الصحية المجتمعية

 توفير رسائل تثقيفية صحية مناسبة مع ثقافة المجتمع حول مواضيع تتعلق بالأمراض غير المعدية ، والوقاية من الأمراض السارية ، والنشاط البدني ، والتغذية ، والصحة العقلية ، والعنف المبني على النوع الاجتماعي ، وصحة الطفل والأم ، والصحة الإنجابية.

 دعم أنشطة تقييم الاحتياجات الصحية.

 إعطاء رسائل صحية في المجتمع في الموضيع المهمه للمجتمع ، مما يجعل العائلات على دراية بالخدمات المتوفرة التي تقدمها IRC أو الجهات الصحية الأخرى.

 قياس التحسن في وعي المجتمع عن طريق إجراء التقيمات القبلية و البعدية بدعم من فريق المتابعة والتقييم.
