The Intensive Case Management (ICM) program provides strengths-based, client-centered case management for refugees, asylees, special immigrant visa holders (SIVs) and other eligible populations who are experiencing barriers to self-sufficiency in the United States. Populations served may include but are not limited to: individuals with medical challenges, individuals with mental health needs, single parents, LGBTQ+-identifying individuals, isolated women and/or women at risk of gender-based violence, survivors of torture or trauma, and elderly individuals. The ICM program provides holistic case management services, frequently focused on healthcare navigation, ensuring that clients are connected to necessary services and resources both within and outside of IRC's programs.The part time ICM Casework Assistant will work 10 hours a week and reports to the Health and Wellness Coordinator.

Major Responsibilities:

  • Conduct telephonic check-ins with active clients using interpretation as needed.

  • Assist with client assessments at intake, 6 months, and 12 months after ICM enrollment.

  • Work in collaboration with the client to develop individualized goals and service plans.

  • Map community assets and help make referrals to community partners.

  • Assist in scheduling appointments and transportation to appointments for clients as appropriate.

  • Assist clients with navigating healthcare and social service systems, including assisting clients with benefits applications.

  • Input case note documentation into IRC's database.

  • Use IRC tools and technology to provide remote case management.

  • Communicate client concerns with ICM staff.

  • Manage a caseload of ICM clients including developing individualistic service plans.

  • Assess and monitor client progress at regular intervals to ensure progress on goals is made and designated resources are maximized, modifying initial service plans and other case file documents as required.

  • Build and maintain partnerships with external community service providers to improve client access.

  • Assist clients with overcoming the barriers they face and achieve their goals for economic and social self-sufficiency.

  • Other duties as assigned.
