Permian Basin SAS Case Worker

Job Overview/Summary: The Permian Basin SAS Case Worker works with unanticipated arrivals to the Permian Basin area to provide social adjustment assistance. This position works close together with Abilene based staff to ensure shared clients receive basic support. The position works from home, with open hours at a partner location in both Midland and Odessa, or alternatively out of the Abilene office.

Responsibilities may include, but are not limited to:

•Provide intake, needs assessment, and enrollment services for clients referred to social adjustment services

•Develop comprehensive service plans in collaboration with clients

•Develop working partnerships for social service providers located in Midland and Odessa, and ensure referral processes for clients are in place

•Assist clients with public benefits applications, including SNAP, Medicaid, WIC, and TANF

•Connect clients with medical providers as appropriate

•Ensure case files and reports are maintained in accordance with grant requirements. 

•Other duties as requested
