Implementation of ICD11 and ICD11 electronic based tools for Medical Certification of Causes of Death (MCCoD) in Angola

Terms of Reference of Consultancy

Background:

The International Statistical Classification of Diseases and Related Health Problems, commonly referred to as the International Classification of Diseases (ICD), is the global standard classification for mortality and morbidity statistics. Such data, broken down by age, sex and cause of death, constitute the foundation of public health.

ICD-11 is designed to meet the needs of diverse users and the demands of information technology. One important innovation is the use of digital tools and platforms for support of coding, translation and testing and the presentation of ICD-11 for use in digital environments.

ICD-11 uses in 35 countries where it has already been implemented include causes of death, primary care, cancer registration, patient safety, dermatology, pain documentation, allergology, reimbursement, clinical documentation, data dictionaries for WHO guidelines, digital documentation of COVID-19 vaccination status and test results.

It is crucial to recognize the importance of developing unconventional methods at the community level to fill information gaps and strengthen national information systems in individual Member States to draw on Their experience in developing and applying community-based health information methods covering health problems, related risk factors, needs and resources.

To facilitate ICD-11 implementation in countries, AFRO has developed a package of ICD-11- based electronic tool for medical certification of cause of death embedded into DHIS2 that countries can adapt. This new tool will accelerate the implementation of ICD-11 in the countries. This tool provides an integrated framework for a standardized analysis and data visualization on deaths and causes of death.

The DHIS2 is implemented national wide in Angola. The country is also using Clinical Platform (SIGHOSP) for hospital management. With the support of partners, the country is working to ensure the interoperability between the two systems.

WHO Regional Office for Africa has received a request from the MoH of Angola to support the implementation of the ICD‐11‐based electronic tool for medical certification of cause of death within their electronic medical records and data reporting systems (DHIS2 and Clinical Platform) to improve the quality and reporting of death and cause of death data.

Objectives and tasks

The objective of the assignment is to support the implementation of ICD-11 and ICD-11-based electronic tool for medical certification of cause of death (MCCoD) (DHIS2 and other platforms) in Angola and provide technical support to the country for the pilot of the tool in selected health 2 facilities. The Consultant(s) will perform the following tasks:

1. Carry out a comprehensive landscape analysis of the DHIS2 and Clinical Platform (SIGHOSP) instances in Angola, including aspects such as governance, integration, system design, and capacity

2. Investigate the compatibility of the existing systems to deploy ICD-11 compliance and the medical certification of cause of death form to inform an operational plan for ICD-11 implementation.

3. Customize the ICD-11-based electronic tool for MCCoD and integrate it with Angola’s DHIS2 instance and other platforms.

4. Support the piloting of the tool in selected health facilities.

5. Conduct the ToT workshop at National level for the implementation of ICD-11 in Angola.

6. Provide a final report summarizing the process and experience. The information should document the knowledge and develop step-by-step guidance for customizing the ICD-11- based electronic tool for MCCoD and the corresponding data forms and workflows integration to Angola DHIS2 instance and other principles clinical platforms used.

Deliverables:

The following are the key deliverables:

1. ICD-11-based electronic tool for MCCoD customized and integrated into Angola DHIS2 and other electronic reporting system.

2. The tool is fully functional in all piloted health facilities private and public.

3. National ICD-11 implementation technical team trained on ICD-11 electronic tools implementation including integration and maintenance to DHIS2 and other platforms.

4. Portuguese implementers and user guides developed.

5. A final report summarizing the process and experience.

Required qualifications and experience:

1. Education:

Essential: A first level university degree in Computer Science, statistics, information systems, or related field.

2. Experience

Essential: At least five years of experience in software development/implementation in health information systems. Solid programming expertise and experience with the development and implementation of DHIS-2 software.

Desirable: Previous satisfactory work with UN agencies and/or NGO in work stipulated in this term of reference.

3. Use of Language skills

Essential: Portuguese

Desirable: Working knowledge of English or French
