1.Purpose of the Consultancy:
Training of Male doctors and other healthcare workers on NCDs and early detection and referral of common cancers
2.Background
Non-Communicable Diseases (NCDs) are a growing public health concern in Afghanistan, contributing significantly to morbidity and mortality. Among these, breast and cervical cancers are leading causes of cancer-related deaths among women. Early detection and timely referral are essential to improving outcomes. However, there is a critical need to build the capacity of frontline healthcare providers, including Community Health Workers (CHWs), midwives, and General Practitioners (GPs), to recognize early signs and symptoms, provide appropriate counseling, and facilitate timely referrals.
To address this gap, WHO is initiating two six-day training programs aimed at strengthening the knowledge and skills of healthcare providers in NCD prevention and control, with a special focus on breast and cervical cancer.
3.Planned timeline:
8 months (including preparation, delivery, and reporting).
Training Duration: 2 training courses each consisting of 6 consecutive days.
The trainings will run with a parallel with 6- day trainings for female nurses, midwives, doctors and CHW trainers
The consultancy will span 8 months part-time with possible extension based on performance and evolving needs.Date |

4. Work to be performed
The primary objectives of this consultancy are:
Outputs:
To design and deliver a 6-days training of trainerâs program for trainers of male nurses and doctors. This will take the form of two separate modules:
NCD Module: Four days TOT on Guidelines for NCDs prevention and Control
Cancer Module:Two days TOT on Cancer including screening, early detection and referral of the most common cancer such as Esophageal, Gastric and Colorectal Cancer as the most common cancers reported in Afghanistan.
1.To design and deliver the two modules of training of trainerâs program for male nurses and Community Health Worker trainers. This will take the form of four days of training on NCDs and Two days on cancer, with an emphasis on screening and early detection of breast and cervical cancers with emphasis on the other most common Cancers such as esophageal, gastric and colorectal cancers.
2. To enhance the capacity of healthcare providers in patient education, evaluation, screening, referral, and follow-up for NCDs and cancers.
5. Deliverables:
The consultant will be responsible for:
·Conducting a rapid needs assessment to tailor the training content.
·Designing a comprehensive training curriculum covering:
oOverview of NCDs and their burden in Afghanistan.
oRisk factors, prevention, and management of major NCDs.
oEarly detection and referral pathways for cancers, especially breast, cervical, esophageal, stomach and colorectal cancers.
oCommunication and counseling skills for community engagement.
·Developing training materials, including presentations, handouts, and job aids.
·Facilitating two six-day in-person training of Trainers workshops in Afghanistan.
·Conducting pre- and post-training assessments to evaluate knowledge improvement.
·Providing a final report with recommendations for future capacity-building initiatives.
6. Final Outcome
The consultant is expected to deliver:
·Training curriculum and agenda.
·Training materials (slides, handouts, job aids).
·Pre- and post-training assessment tools and results.
·Final training report including participant feedback and recommendations.
Month 1:
Conduct situation analysis and stakeholder consultations.
Develop training materials and draft clinical workbooks.
Finalize training materials and workbooks.
Month 2:
Deliver first training of trainersâ workshop (male doctors and nurses) in the primary care prevention and management of major NCDs and early detection, referral and follow-up of common cancers in primary care including simulated practical components.
Months 3-4:
·Review lessons learned from the ToT workshop and make any necessary adjustments to the training materials (slides, handouts, job-aids)
Months 5-6
Deliver second training of trainers workshop (male nurses and CHWs) in prevention and community management of major NCDs and prevention, early detection and referral of common cancers
Months 7-8:
Review lessons learned and make any necessary final adjustments to training materials (slide deck, handouts, job-aids)
Produce final report with recommendations

8. Specific requirements
Qualifications required:
·Masterâs degree in public health and or medicine.
Experience
Essential
·From 5 to 10 years of experience in NCDs management and treatment, cancer control and/ or health education.
·Demonstrated expertise in designing and delivering training programs for healthcare providers, preferably in fragile or low-resource settings and specific to Afghanistan.
·International Experience is a requirement for international consultancies.
Desirable
·Knowledge of the Afghan health system and cultural context is considered an asset.
Skills / Technical skills and knowledge:
·Strong facilitation, communication, and report-writing skills.
9. Place of assignment
The consultant will perform his duties in a hybrid manner with most of his assignment to be performed remotely from his home country and visit Kabul twice for two weeks to facilitate TOTs physically
10. Medical clearance
The selected Consultant will be expected to provide a medical certificate of fitness for work.

All travel arrangements will be made by WHO â WHO will not be responsible for tickets purchased by the Consultant without the express, prior authorization of WHO. While on mission under the terms of this consultancy, the Consultant will receive subsistence allowance.
