 Project Context and Scope


Thailand has achieved Universal Health Coverage for Thai citizens and maintains several health insurance schemes that include migrants; however, significant challenges persist in ensuring equitable access to healthcare for migrants of all genders and ages. The ongoing crisis in Myanmar has resulted in complex and continuous migration flows into Thailand, with more than 1.2 million migrants entering the country in 2024 alone. Tak province hosts the largest formal border crossing point (Myawaddy–Mae Sot), making it one of the most heavily affected areas in terms of migration‑related public health pressures. The increasing influx of migrants has intensified the disease burden in border areas and expanded the strain on health facilities, medical personnel, and provincial budgets. Mae Sot General Hospital reported non‑Thai patients represented 34% of total cases in 2023, and health service expenditure for migrants in the first four months of 2024 almost equaled the total expenditure for all of 2023. Expenditure shortfalls rose from 14.19% in 2023 to 36.65% in 2024, underscoring critical health system financing and sustainability challenges.

Migration health challenges in Tak province include maternal and child health issues, vaccine‑preventable disease risks, TB and HIV continuity‑of‑care gaps, malaria prevalence (with 95% of all malaria cases in Thailand occurring in border regions), and the heightened risks posed by displacement and poor living conditions. The December 2024 cholera outbreak—which spread into Tak province—further highlighted the urgent need to strengthen provincial preparedness and cross‑border coordination systems.

Fragmented governance structures contribute to inconsistent approaches to migrant health. The absence of fully harmonized coordination between government agencies, NGOs, private sector partners, and border‑area actors results in unclear referral pathways, underutilization of insurance schemes, inconsistent data systems, and gaps in gender‑responsive and rights‑based policy implementation. Women migrants, children, undocumented migrants, persons with disabilities, and individuals of diverse SOGIESC identities face layered vulnerabilities and disproportionate barriers in accessing essential healthcare services.
In response, this project aims to contribute to the Government of Thailand’s efforts to enhance migration health governance through an adaptable and scalable provincial approach for improved access to health services, with Tak province serving as the pilot. The project focuses on establishing inclusive, rights‑based, and sustainable migration health governance mechanisms by:


•    conducting a stakeholder analysis,
•    implementing a gender‑sensitive migration health system governance assessment,
•    establishing a provincial Migrant Health Working Group,
•    strengthening public health emergency preparedness through simulation exercises and cross‑border coordination, and
•    improving the capacity and sustainability of the Migrant Health Volunteer (MHV) system.

These efforts will feed into the development of an evidence‑based, gender‑sensitive provincial Migration Health Action Plan, aligned with the emerging National Migration Health Strategic Plan, and will contribute to replicable tools and frameworks for nationwide scale‑up.

The purpose of this consultancy is to strengthen the provincial Migrant Health Volunteer (MHV) engagement system through the review and formalization of a standardized, competency based training curriculum and an accompanying performance management framework. The consultant will review existing MHV training materials, delivery modalities, and supervision practices in Tak Province; identify gaps and inconsistencies in content, competencies, and performance monitoring; compare it with other existing curricula across Thailand and globally; and translate these findings into practical, gender  and rights sensitive tools that can be institutionalized by provincial actors. The assignment aims to ensure that MHVs are equipped with clearly defined roles, competencies, and performance expectations that enable them to effectively support migrant access to health services and community based health surveillance within the provincial health system.

While a range of global and national approaches exist for community health worker training, competency development, and performance management, these frameworks are often designed for national programmes and do not sufficiently reflect the operational realities of migrant inclusive service delivery in border and mobility affected settings. This consultancy will therefore focus on adapting relevant good practices and evidence based models for community health volunteer training and performance management to the specific context of Tak Province, in close coordination with the Tak Border Health Learning Centre (BHLC), Provincial Health Office (PHO), and Migrant Health Working Group (pMHWG). In line with the reviewed and standardized curricula, training sessions will be conducted for selected MHVs which will be sustained through stipends and financial contributions from this project. The consultant will also work on developing Standard Operating Procedures (SOPs) to support community-based health surveillance among migrant communities, and lead discussions with all stakeholders on MHVs sustainability, including the mapping of organizations working with MHVs, and the potential establishment of a MHV registration system in Tak province and MHVs fund for financial sustainability.

Building on ongoing project activities and complementary assessments under the project, the consultant will support the development of a comprehensive provincial MHVs system that is locally relevant, implementable, and aligned with provincial and national health priorities, thereby contributing to more sustainable and inclusive migration health governance; and that can be expanded later to include additional provinces.

Responsibilities

Tasks to be performed under this contract
1.    First Deliverable – Inception Report:
  • Includes methodology, workplan, stakeholder consultation plan, proposed outline and initial review of the current MHVs training curriculum by Tak BHLC.
  • First instalment payment – 20%.
  • Delivered by: 1 July 2026.

2.     Second Deliverable – Standardized Curriculum and Performance Management Framework:
  • Includes:
    o    Reviewed and consolidated gender‑ and rights‑sensitive MHV training curriculum;
    o    Standardized training module outlines and learning objectives;
    o    MHV competency framework linked to roles, skills, and responsibilities;
    o    Performance management framework, including monitoring indicators and reporting tools;
    o    Technical inputs to support community‑based health surveillance linkages.
    o    Lead the facilitation of meetings on MHVs fund / financial sustainability discussions
  •  Second instalment payment – 50%.
  • Delivered by: 31 October 2026.

3.    Third Deliverable – Final Package:
  • Includes:
    o    Finalized MHV training curriculum validated with BHLC and IOM;
    o    Final MHV competency and performance management framework;
    o    Practical recommendations on MHV sustainability (MHVs fund), supervision, and incentives;
    o    Inputs for inclusion in the project replication toolkit;
    o    Final presentation of findings and tools to IOM, BHLC, PHO, and partners.
  • Third and final instalment payment – 30%.
  • Delivered by: 31 November 2026
Performance indicators for the evaluation of results
1.    Timeliness: All deliverables (inception report, draft assessment, final report, presentation) submitted within agreed timelines.
2.    Technical Quality: Assessment demonstrates methodological rigor, reflects project RBA/gender standards, and integrates stakeholder perspectives.
3.    Relevance & Accuracy: Findings align with provincial and national migration health priorities and accurately reflect data collected in Tak province.
4.    Usefulness: Recommendations are actionable, evidence‑based, and directly support the development of the provincial Migration Health Action Plan.
5.    Stakeholder Engagement: Effective coordination with MoPH, PHO, BHLC, partners, and migrant communities, ensuring inclusive participation.
 

Qualifications

Required Qualifications and Experience

Education

  • Master’s degree in Public Health, Health Systems Management, Social Sciences, Migration Studies, Public Policy, or a related field. A PhD in a relevant field is a strong advantage.
  • Accredited Universities are those listed in the UNESCO World Higher Education Database.
     

Experience

  • Minimum 10 years of professional experience in migration health programming, community health, health systems strengthening, health governance, or public health policy analysis.
  • Demonstrated experience working on Migrant Health Volunteers capacity building.
  • Proven experience working with government health authorities, UN agencies, and INGOs.
  • Experience working in border health or migrant health contexts.
     

Skills

  • Strong analytical, report‑writing, and facilitation skills.
  • Ability to work independently and coordinate with multiple stakeholders.
  • Experience in participatory training design and facilitation;
  • Knowledge of Thai health system and migrant health context is an asset.
  • Fluency in English and Thai languages is required
     

Languages

Fluency in Thai and English (oral and written) is required. 

Travel Required

Should the Consultant be required to travel by the Organization, the Consultant will be provided with transportation or reimbursed for the cost of their travel following advance agreement and in accordance with IOM’s travel policies, including IN/6. The Consultant will be entitled to a Daily Subsistence Allowance (DSA) at IOM’s established rates during their temporary duty (TDY) at the designated duty station.

  • The consultancy is home-based. Duty station is Bangkok, Thailand, with multiple field visits to Mae Sot, Tak province required.
  • Participation in meetings with MoPH and partners in Bangkok maybe required.
