The establishment of the Program of Medical Guarantees (PMG) and the National Health Service of Ukraine (NHSU) introduced strategic purchasing as a cornerstone of the health system, creating a growing demand for robust analytical evidence to inform provider payment reforms, budget planning, and resource allocation.WHO has been supporting the Ministry of Health (MoH), the NHSU, and other national institutions in strengthening strategic purchasing through health service costing, provider payment design, financial modelling, and health expenditure analysis. Since 2021, WHO has supported three comprehensive costing studies of primary health care (PHC) services, covering the core PHC package as well as mental health, tuberculosis, HIV, opioid agonist therapy, and mobile palliative care services. This work resulted in the development of Ukraine's first comprehensive methodology for calculating the PHC capitation rate, which has been used by the NHSU to inform annual adjustments to the PMG budget. Building on these achievements, WHO is supporting further reforms related to the financing of essential PHC services, including the development of technical guidance on PHC costing, budget estimation methodologies, and capacity building for NHSU staff.WHO is also supporting the Government of Ukraine in strengthening health financing analysis in other priority areas. Since 2020, WHO has collaborated with the State Statistics Service of Ukraine (Ukrstat) to improve health expenditure tracking through the production of National Health Accounts based on the internationally recognized methodology System of Health Accounts 2011 (SHA 2011). In parallel, WHO is supporting the MoH and the NHSU in analysing the financial implications of implementing cross-border healthcare arrangements as part of Ukraine's accession to the European Union, including the development of financial models and budget impact scenarios. Support mapping of 2024 health expenditure data according to SHA 2011 and contribute to the production of National Health Accounts 2024 by 30 September 2026.Deliverable 2. Develop detailed technical instructions for the NHSU on conducting PHC costing studies and calculating the capitation rate by 30 October 2026.Deliverable 3.
