Community Navigator
Location: Covering county of Denbighshire (On-site)
Salary: £25,541 to £27,200 per annum, pro rata
Hours: 35 per week
Contract: Fixed term contract (until 31st March 2027)
Additional Requirement: Applicants must have a full licence (2 years) and access to a vehicle.

Our team of Community Navigators in Denbighshire provide a link between Health and Social Care professionals, Denbighshire citizens, their family, and carers. As part of the team, you would provide assistance within the community and third sector. This approach can also be known as ‘Social Prescribing’ which is a mechanism for linking citizens with non-medical sources of support within the community.
You will recognise that people are experts in their own life but sometimes they need someone else to help them to identify and work towards the outcomes that they would like to achieve.
What will a day in the life of a Community Navigator involve?
- You will work closely with the Community Resource Teams, ‘Talking Points’, SpoA Wellbeing Coordinators, Complex Disability Team, Primary care, Community Mental Health Teams and other people from a range of local services.
- You will engage with service users of Talking Points in their normal place of residence or perhaps at another setting. You will carry out needs assessments and develop support plans utilising the “What Matters” approach.
- You will be a source of current, accurate and timely information and knowledge about a comprehensive range of help available within the community. You will improve the service quality for the benefit of service users.
What do you need to be a successful Community Navigator?
- Being proactive, can react quickly to service requests and appreciate the importance of the service you are providing.
- Dealing with queries in a diplomatic, confidential and non judgemental manner.
- Comfortable planning your own workload and working remotely whilst still part of a team.
- Excellent communication skills, both written and verbally.
- IT literate, particularly with Microsoft 365 packages
