While pulmonary TB constitutes the majority of reported cases, extrapulmonary tuberculosis (EPTB) accounts for a substantial proportion of notified TB cases in several countries and presents unique diagnostic, surveillance and programmatic challenges.The WHO Global TB database collects information on TB case notifications and treatment outcomes, including disaggregation by TB case type, age, sex, HIV status and drug resistance. These data provide an opportunity to better understand trends and patterns of EPTB across the Region. According to WHO’s 2025 Global Tuberculosis Report, EPTB accounted for 23% of notified new and relapse TB cases in the Eastern Mediterranean Region in 2024, compared with 16% globally. WHO notification data are reported annually by national authorities using standard definitions and recording and reporting guidance.Countries supported through the current (GC7) Global Fund Middle East Response (MER) grant, namely Iraq, Jordan, Lebanon, Libya, Palestine, the Syrian Arab Republic and Yemen, operate in contexts affected by humanitarian emergencies, conflict, population displacement and health system disruption. These circumstances may influence the epidemiology, diagnosis, treatment and reporting of EPTB.WHO EMRO seeks to undertake a regional desk review to describe the epidemiology and programmatic implications of EPTB in the Eastern Mediterranean Region, while providing an in-depth analysis of EPTB in MER-supported countries. The review will use routinely reported notification data, national programme documents, Global Fund reports and selected published literature to describe EPTB notification patterns in MER-supported countries, compare these with regional trends, and identify practical implications for surveillance, diagnosis and programme investments. The findings will support regional policy dialogue, programme planning, technical assistance and investment priorities for the Global Fund and donors/domestic financing, and future operational research.Work to be performedObjective:To assess the epidemiology, trends, determinants and programmatic implications of extrapulmonary tuberculosis in the Eastern Mediterranean Region.1. Review available evidence on demographic and clinical characteristics of EPTB in the selected countries.5.
