Background: The International Rescue Committee helps people whose lives and livelihoods are shattered by conflict and disaster to survive, recover, and gain control of their future.

Scope of Service: The Healthy Futures AmeriCorps member will partner with the Family Support team to address barriers to health, wellness and self-sufficiency for refugees and other immigrants with a focus on physical and mental health issues. This position reports to the ICM Caseworker .

Responsibilities:

•Coordinate and lead workshops and group activities on topics related to health and wellness

•Conduct health and wellness assessments for clients enrolled in the Intensive Case Management (ICM) program

•Assist with the development of self-sufficiency plans based on client needs; monitor progress toward goals

•Conduct regular check-ins with active clients

•Map community assets and help make referrals to community partners

•Assist in scheduling and accompanying clients to health and wellness appointments as appropriate

•Assist clients with navigating healthcare and social service systems

•Record client interactions and services in IRC’s electronic database

•Communicate client concerns with Family Support staff and coordinate health and wellness service plan with supervisor

•Meet regularly with supervisor and Family Support team

•Assist with the management of volunteers and interns for the Family Support program

•Assist in rolling out client satisfaction surveys, compile the results, and provide findings to the program supervisors

•Participate in all program meetings, development activities, and other meetings as needed
