Undertake regular hand holding and support to implementation of field level health program activities.
⢠Coordinate at Panchayat and block level for convergence of sources.
⢠Ensure baseline & midline health data collection as per timeline
⢠Closely work with Community and anganwadi centers.
* Mobilize the community for implementation of primary health Services and nutrition care.
* Mobilizers (BCMs) and ASHA Facilitators to strengthen the VHSND functioning in terms of:
* organization of community meetings on a regular basis,Monthly basis.
* community based monitoring for health project and related work.
* Support organisation of RKS training, follow ups in health camp
and support in organisation of regular meetings with Sahiya and ASHA workers.
* Facilitate organisation of Block level Planning.
* Work with the Block Programme Manager and
Block Community Mobilizer on regular review of data from , HMIS, FPLMIS and issuing directives/ communication with front line
providers.
*Participate and contribute at the regular block
and PHC level meetings.
* Prepare notes & Reports from the field Level.
