Background of the project 

IRC is the lead health and nutrition implementing partner in Kakuma and Hagadera refugee camps, serving both Persons of Concern (POCs) and the host community with an integrated package of primary health care (PHC) services including outpatient consultations and inpatient care for communicable and non communicable diseases; provision of reproductive, maternal, newborn, child, adolescent health and gender based violence services; screening and treatment for acute malnutrition through a community approach for non-complicated cases and for those with medical complications through inpatient care in stabilization centers; and community based surveillance for diseases of public health importance and health promotion and prevention through a robust community health program at the camp level.

This is delivered through the operation of ten health facilities in total in the two camps. Seven in Kakuma, which are, one level 4 - Ammusait General hospital, one Level 3 – Kapooka Main hospital and four level 2 dispensaries: Hong Kong, Locherang’amor, Nationokar and Nalemsekon and a reception center at which medical screening is done for the new arrivals. In Hagadera, three health facilities are operated by IRC: one level 4 – Hagadera hospital, and two health posts - L6 and E6 in Hagadera camp.

Historically, health and nutrition service delivery in the refugee camps has been heavily donor driven with direct service delivery in line with humanitarian response guidelines. Program implementation is through siloed approaches with stand alone service delivery points for health and nutrition, an approach that has eroded integration and is quite expensive. Overtime, this is changing, starting with registration of the health facilities in the camps by the Ministry of Health, linkage to the government supply chain system for commodities under the Nutrition, HIV/AIDS, Tuberculosis, Malaria, and family planning programs.With the proposed social integration of refugees in their host communities in line with the SHIRIKA plan, and with the elevation of Kakuma and Daadab to municipalities, there is an urgent need for a review of the health service delivery model within the camps to align it to the government plans and the Ministry of Health integrated service delivery model for both facility and community services.

IRC will integrate Nutrition services within the existing health service delivery points in MCH, under 5 OPD clinics, pharmacy inlets and outlets and at community level by doing away with the stand-alone service delivery points within the health facilities. Being a new model and approach, its adoption requires a structured implementation road map to carefully manage the change process to ensure that all stakeholders involved in the transition fully understand the model and approach, their roles, and responsibilities, and fully commit to implementing it to make it a success and that implementation is monitored periodically to review progress as well as inform learning and reprogramming.

Objectives:

  1. IRC Kenya is seeking the services of a dynamic Consultant to provide technical oversight in the development of an implementation roadmap for the adoption of an integrated health service delivery model for its health and nutrition programs in Kakuma and Daadab refugee camps. The process will
  2. Assess the capacity of IRC health facilities and community health system to establish readiness for integration - in terms of infrastructure, equipment, patient flow, leadership & governance, staffing and staff skills.
  3. Review the organizational program management structure of the IRC healthprogram against the Ministry of Health best practice for hospital management systems.
  4. Propose an integrated model of service delivery that will enhance efficiency and effectiveness in health service delivery.
  5. Adopt a management structure that will enhance accountability for integration.
  6. Conduct stakeholder mapping and analyze their roles and responsibilities in integration.
  7. Develop a service integration road map to guide the process of service integration.
  8. Develop a framework for monitoring and evaluation of the integrated service delivery model for learning and improvement.

Scope of work

The consultant will: -

  1. Conduct a comprehensive desk review of the current service delivery models for health and nutrition at both health facility and community level.
  2. Conduct key informant interviews and focused group discussions with IRC staff and key stakeholders.
  3. Conduct field visits to the IRC health facilities, communities, and stakeholders.
  4. Review the existing health program and hospital management structures.
  5. In health facilities where integration has been carried out, assess the immediate lessons learned.
  6. Analyze the proposed Integrated health and nutrition model to clearly define the points of intersection for service delivery at both community and health facility level.
  7. Conduct stakeholder mapping and define clear roles and responsibilities for each.
  8. Triangulate findings from the desk review and KIIs to identify and highlight some of the barriers, opportunities, and potential strategies for adopting and implementing an integrated health service delivery model in the refugee program.
  9. Facilitate a writing workshop for the IRC integration task force to develop the road map.
  10. Develop a detailed priority implementation framework for the road map including a monitoring and evaluation plan.
  11. Develop an orientation package and plan for IRC management, health facility and community level staff on the integrated service delivery model.
  12. Assess the Cost implication of the integrated service delivery model and develop a costed budget for the implementation of the road map.

Deliverables:

  1. A costed road map.
  2. Stakeholder mapping matrix.
  3. Staff and stakeholder training plan.

Qualifications

  • A health professional with post graduate degree in Public health, Health Economics, Social science or a related science field and a practicing license that permits the clinical practice in Kenya for regulated professions.
  • Proven experience in reviewing health service delivery models and proposing integrated approaches that have led to improved efficiency and effectiveness.
  • At least 10 to 15 years’ of progressively increasing responsibility working on Integrated health service delivery models.
  • Having at least 5 years’ experience in a senior leadership position that provided technical guidance to development of improvement plans in health service delivery will be an advantage.
  • A very good knowledge and understanding of the Kenya Ministry of Health service delivery model.
  • Familiarity with Government of Kenya health policies, priorities and evidenced-based practices aimed at improving the quality of and access to health services for all.
  • Experience in interacting with national and county government, development partners, civil society, and community-based organizations at different levels.
  • Demonstratable excellent technical writing skills
  • Demonstrated oral and written communication skills, and the ability to develop and deliver technical presentations.
  • Demonstrated ability to interpret research findings and apply them to the Kenyan context when required.
  • Fluency in English and Swahili
  • Prior experience developing strategic plans, workplans, and/or activity plans for health care service delivery programs.

Skills and competencies

  • Excellent skills in quantitative and qualitative data collection and analysis. i.e., FGD
  • Excellent organizing, facilitating, presentation, and communication skills, including good report writing in English.
  • Experience of working with Govt., communities, INGOs, and donor agencies. Strong understanding of relevant networks and consortiums.
  • Strong understanding of budget development and management.
  • Flexible approach to meet changing and emerging needs as appropriate.
