Background
  1. The Ministry of Health has developed and implemented testing and treatment services guidelines to strengthen efforts to advance provision of quality HIV, STIs and hepatitis services in order to meet the global targets by 2020.
    To strengthen quality of HIV, STIs and hepatitis testing and treatment services and finding innovative way to help achieve the national targets, Universal Health Coverage (UHC), as well as the Sustainable Development Goals (SDG) a review of the existing national testing and treatment guidelines or structures is planned for 1st November -8th December, 2018.The development of new guidelines will ensure that all the proposed recommendations are in line with the latest WHO global guidance for HIV, STIs and viral hepatitis testing, treatment and care.
  2. The overall objective
    To provide technical support to develop a guideline within which HIV, STIs and hepatitis testing and treatment can be implemented safely, effectively and accurately to enhance the quality delivery of HIV, STIs and Hepatitis programmes and services in Seychelles.
    To achieve the objective, a consultant will be recruited to provide support to this process from 1 Nov-8 December 2018
  3. The specific objective will include
  • Review and update the comprehensive guidelines related to HIV, STIs and Hepatitis testing and treatment that will enhance health system efficiency by focusing on health services to ensure that the provision of testing and access to treatment are in line with WHO standards.
  • Outline programmatic approaches to HIV, STIs and hepatitis testing and treatment.
  • Describe commodity management systems required for HIV, STIs and hepatitis testing and treatment
  • Outline quality assurance strategies in HIV, STIs and Hepatitis testing and treatment.
  • Outline the coordination monitoring and evaluation strategy for HIV, STIs and hepatitis testing and treatment mechanisms.
 
  1. Expected outcome
  • The updated comprehensive guidelines for HIV/Hepatitis and Syphilis in line with WHO latest guidance covering
    • Identification of programmatic approaches to HIV, STIs and Hepatitis testing and treatment that is required to support and enhance quality laboratory and treatment service delivery.
    • Recommendations on commodities management system required for HIV, STIs and hepatitis testing and treatment.
    • Quality assurance strategies developed for HIV, STIs and hepatitis testing and treatment.
    • Coordination, monitoring and evaluation strategies developed for testing and treatment.
  1. The TORs of the mission
  • Review the existing testing and treatment draft documents related to HIV, STIs and Hepatitis.
  • Review the WHO scoping mission report to understand the HIV/Hepatitis testing and treatment issues observed and recommended
  • Meet and discuss with technical working groups and key informants including the CSOs and people living with HIV/Hepatitis to identify issues related to HIV, STIs and Hepatitis.
  • Assess the existing testing and treatment coordination, monitoring and evaluation systems for the laboratory and the treatment management and its linkage to the broader Health management information system.
  • Conduct selective site visits including the HIV/Hepatitis laboratory and treatment centres and assess the national laboratory and treatment capacities to appreciate the ground realities that will enrich guidelines development and service delivery.
  • Review the existing HIV testing algorithm (for HIV/Syphilis and Hepatitis separately) and treatment protocol and quality measures implemented in the HIV and treatment testing sites including the use of HIV self-testing and the combination ART tablet.
  • Assess the available viral load/EID testing algorithm, Laboratory Quality manual, testing validation reports and provide recommendations on how to optimise the VL/EID testing including the use of POC.
  • Review the available laboratory capacity for diagnosis of HBV and HCV and available testing algorithm and provide recommendations on the use of diagnostic tests (including rapid and combined tests) for HBV and HCV testing.
  • Based on identified gaps through review of drafts guidelines, site visits, technical working group discussions and recommendations from the scoping missions, draft/update the comprehensive testing and treatment guidelines for HIV/Hepatitis and Syphilis.
    Conduct a validation workshop on the guidelines with MOH team and other national stakeholders.
     
  1. QUALIFICATIONS
    1. Education
  • The preferred candidate should have a university degree in Medicine (MD) or other health sciences, health management, and a master's degree in public health, epidemiology, from recognized university.

    6.2. Desirable\:
    Health Policy and Planning including National Strategies and guidelines development, Monitoring and Evaluation of Health programmes. The candidate should have
    • Ability to lead policy and strategic planning process
    • Strategic planning skills
    • Project planning and management skills
    • Good communication, interpersonal and presentation skills
    • Ability to work under limited, broad direction and meet deadlines.
    • Working experiences in development similar guiding documents for HIV/Syphilis and viral hepatitis
    • Proven leadership, management, facilitation, coordination, interpersonal, and teamwork skills.
    • Sound judgment, tact and diplomacy, and proven ability to work under pressure to meet deadlines or dates for special events.
    • Experiences in HIV/Hepatitis programming at National and International level, with strong bias on strategic programme development and reviews, and health service delivery mechanisms in developing countries
  1. Work Experience\:
  • A minimum of five years of progressive and responsible experience in strategic planning, programme/project and health service design, delivery, management, monitoring and evaluation
  1. Knowledge
  • Knowledge and understanding of HIV, hepatitis and sexually transmitted infections and their personal, social and public health impact
  • Good understanding of the complex interface between the government and various health programmes and sectors, non-government, partners, donors and community sectors
  1. Languages
    Very good knowledge of English and knowledge of French will be added advantage
  2. Supervision\: With support from AFRO/IST, the consultant will be reporting directly to WHO Represented
 
  1. Duration- 5 weeks (1 Nov-8 December 2018)
1\: First week- in country consultation
2. Second week- development/updates of the guidelines (offshore)
3. Third week- submission of the draft guidelines and circulation of the draft to key stakeholders
4.Weeek 4\: in country facilitation of guidelines validation workshop
5 Week 5\: consolidating the guidelines with all inputs and submission of final version
  1. Remunerations
  2. Consultancy fee\: The consultant will be paid at P4 level; $9000 Per Month under the below payment arrangements
  • 25% on signing;
  • 50% after concluding the guidelines validation workshop
  • 25% on submission of the final guidelines version
    2. Per diem\: Apart from consultancy fee, the consultant will be paid per diem as per UN rate in days that will be in country
