Purpose of the Assignment

To lead the process of developing Water, Sanitation and Hygiene Standards for Health Facilities in Timor-Leste.

Assignment Tasks

1. Conduct literature review, including among others study of a) Relevant national laws, plans and strategies related to WASH, b) the National Health Sector Strategic Plan, c) the National Comprehensive Primary Health Care Guidelines, d) Essential Environmental Health Standards in Health Care (WHO 2008), e) quality measures of adequate water, sanitation and hygiene of ‘Standards for Improving Quality of Maternal and Newborn Care in Health Facilities (WHO 2016); and exchange relevant country background and development of inception report and work plan

2. Conduct country stakeholder’s consultations and field visits and develop background paper with status of health facility WASH standards, strengths, weakness and opportunities for improvement and recommendations for developing WASH facility Standards for hospitals, CHCs and HPs. This will include:

1. In-country consultation  with key technical staff of MoH department responsible for health facility construction and maintenance, Primary Health Care and Environmental Health Department , DGAS MoPWTC, WHO, other key WASH and health sector partners
2. Jointly with MoH established team, visit to a sample of 36 Health facilities (6 Hospitals, 15 CHC and 15 Health posts) and assess the status of WASH facilities using check-list of Essential Environmental Health Standards in Health Care (WHO 2008) and consult local stakeholders to establish the status of health facility WASH standards, strengths, weakness and opportunities for improvement
3. Visit selected nearby public WASH facilities, and completed and on-going project sites close to the health facilities
4. Through the field visits and consultations identify technologies in use including solar water pumping system, rainwater harvesting system, incinerator, etc, needs, gaps and develop a brief background paper with current status of health facility WASH standards, strengths, weakness and opportunities for improvement and recommendations for developing WASH facility Standards for different levels of CHCs and HPs.

3. Develop an outline for developing Draft WASH Standard for different levels of health facilities, Hospitals, CHCs and Health Posts in Timor-Leste based on the findings of consultation and field work. The outline should:

1. Include the following elements of Environmental Health in Health Facilities:

1. 1. • Water facilities, access to water, quantity and quality of water for drinking, cleaning/washing, and other purposes
      • Hygiene Facilities and information on hygiene promotion
      • Toilet Facilities/Excreta disposal
      • Wastewater disposal
      • Health-care waste disposal
      • Cleaning and laundry
      • Food storage and preparation
      • Control of vector-borne disease
      • Other environmental Health consideration for Hospitals, CHC and Health Post building design, construction and management

    b. Show how technical descriptions, preferred design option, alternate design option, and Bill of Quantity (BoQ) of each of the preferred and alternate option of WASH facilities for Hospitals, CHCs and Health Posts will be included in the draft document.

4. Conduct initial stakeholder workshop with key MoH, MoPWTC, Ministry of State and Administration, WHO, and other WASH and health sector partners to share:

1. 1. Background paper with status of health facility WASH standards, strengths, weakness and opportunities for improvement and recommendations for developing WASH facility Standards for Hospitals, CHCs and HPs.
   2. Outline for developing Draft WASH Standard for different levels of Hospitals, CHCs and Health Posts in Timor-Leste

5. Finalize the background paper and outline for developing WASH Standard for different levels of Hospitals, CHCs and Health Posts in Timor-Leste taking in stakeholders feedback

6. Working closely with Ministry of Health Environment Health Department, Public Health Directorate and Chief Administration and Logistic Department of Ministry of Health, develop Draft WASH Standard for different levels of Hospitals, CHCs and Health Posts in Timor-Leste covering all elements of Environmental Health in Health Facilities (using the outline)

7. Working closely with Chief Administration and Logistic Department of Ministry of Health DGAS, MoPWTC,  department of Ministry of Public Works, Transport and communication, develop technical descriptions, preferred design option, alternate design option, and Bill of Quantity (BoQ) of the preferred and alternate option for CHCs and Health Posts. The alternate options should consider appropriate and sustainable technological solutions such as solar pumping or rainwater harvesting or gravity-fed water systems for water supply and relevant appropriate options for waste-management, waste-water management etc. Engineering and technical design engineering expertise from the a Swedish Construction Company called NCC (UNICEF’s Donor) are expected to participate in the work process and contribute to the work, specifically in the development of design options for WASH in Health Care facilities.  

8. Conduct workshop with key stakeholders to finalize the Draft WASH Standards and the technical design options for WASH facilities for different levels of Hospitals, CHCs and Health Posts

9. Finalize the WASH Standards and the technical design options for WASH facilities for different levels of Hospitals, CHCs and Health Posts

10. Present the WASH Standards and the technical design options for WASH facilities for different levels of Hospitals, CHCs and Health Posts to the Ministry of Health Council of Directors

11. Submit final report of the technical assistance

Expected Deliverables

1. Inception report with overview of key consideration / criteria for consideration, and work plan (5 days)

2. Background paper with status of health facility WASH standards, strengths, weakness and opportunities for improvement and recommendations for developing WASH facility Standards for Hospitals, CHCs and HPs (25 days)

3. Outline for developing Draft WASH Standard for different levels of Hospitals, CHCs and Health Posts in Timor-Leste (5 days)

4. Workshop report with stakeholder feedbacks (5 days)

5. Final the background paper and outline for developing WASH Standard for different levels of CHCs and Health Posts in Timor-Leste 93 days)

6. Draft WASH Standards, with a menu of preferred and alternate technical design options for WASH facilities for different levels of CHCs and Health Posts and Bill of Quantity (BoQ) of the recommended and alternate options (15 days)

7. Workshop report with stakeholder feedbacks (5 days)

8. Final WASH Standards and the technical design options for WASH facilities for different levels of Hospitals, CHCs and Health Posts in MS Word document along with a PowerPoint presentation (5 days)

9. Product of the consultancy (technical assistance) which include the following presented in as soft copies in editable format and as PDF files (2 days):

1. Background paper with status of health facility WASH standards, strengths, weakness and opportunities for improvement and recommendations for developing WASH facility Standards for Hospitals, CHCs and HPs
2. Final WASH Standards and the technical design options for WASH facilities for different levels of Hospitals, CHCs and Health Posts.

Qualifications of Successful Candidate

• Advanced University Degree in Civil or Environmental Engineering or other Public Health Engineering or Environmental Health related field;
• At least 8 years of experience in the WASH sector;
• At least 3-4 years at a senior level working in the areas of strategic planning for facility development and planning, programme management and policy and strategy development;
• Experience in developing Standards, Strategies or Guidelines related to Environmental Health preferably standards and design of WASH services for health care services delivery points
• Familiar with WHO recommended minimum standards for WASH services for public health facilities and knowledge of National Building Code/ national standard development / approval processes
• Familiar with the current development, research, best practices and global trend in the area of WASH
• Working experience with UN, INGOs and Government in research & strategy development;
• Knowledge and understanding of WASH facility design, construction and school or community level management would be an asset;
• Working experience in Timor-Leste or in the region is desired.
• Excellent communication and report writing skills in English language
• Experience of communicating using interpreter is necessary for the international consultant/consultants. The international consultant is expected to team up with a National consultant and engage with the national consultant for oral and written translation as needed.
