UNICEF Timor-Leste Country Office is currently looking for a qualified national individual consultant to Establish Continuous Quality Improvement Process in Health Care Facilities with focus on Maternal, Newborn and Child Health.

BACKGROUND

The Timor-Leste Strategic Development Plan 2011-2030 provides a road-maps for developing a decentralized health care system that reaches every community with a defined package of health services. As recently as 2012, communities accessed health services through monthly integrated community outreaches called SISCa, and through fixed Community Health Centers at the Administrative Post level, five referral hospitals, and one national hospital. In recent years, the health services delivery network has been expanded and as of now, all 442 Suco (a group of villages) have a doctor assigned to them. Out of these 313 have Health-Post providing basic services, and all 69 Administrative Posts have a Community Health Centre. These lower level services delivery system is supported by five regional referral hospitals for higher level care with a provision of referring citizens needing referral care outside the country too. A Comprehensive Service Package for Primary Health Care (CSP-PHC) which includes “Saude na Familia” was launched in 2015. The CSP-PHC defines the services to be provided at each level of health care, the necessary technical guidelines, the human resources, supplies and equipment needed to provide those services. Recognizing the importance of engaging community and promoting healthy behavior and practices, the CSP-PHC makes the Health-Post-Service teams responsible for engaging communities in all aspect of health. The Saude Na Familia (family health) program has a domiciliary visit by health professional teams bringing primary health care to the people’s homes, making it accessible to the vulnerable, poor, marginalized, disabled and elderly as well as.

PURPOSE OF ASSIGNMENT:

The overall objective of the consultancy is to assist the Ministry of Health (MoH) and the international consultant establish a Continuous Quality Improvement (CQI) Process as part of core Practice of Health Care Management in the country; and through the CQI process, enable Health Facility Leaders (HFL) and Health Workers (HW) to:

  • Re-organize patient care to ensure quality care,
  • Make efficient use of available resources,
  • Engage in a process of continuous learning to enhance clinical skills, client relation skills, and management skills;
  • Engage in an on-going process of self-assessment to identify problems, developing and implementing self-assessment guided solutions, reviewing progress and learning from it; and
  • As and when needed, communicate effectively with Municipality or National Level sector managers and partners to secure additional resources/support to implement solutions.

The specific objectives of the consultancy are to:

  • Assist the international consultant to develop and translate the Continuous Quality Improvement Approach and a Roadmap for Public Health Facilities in Timor-Leste into the Tetum language;
  • Assist the international consultant to develop/adapt appropriate Continuous Quality Improvement Guidelines and Coaching manuals and facilitation Manuel and quality assessment tools and translate these into the Tetum language;
  • Work with MoH to organize and facilitate (co-facilitator) the CQI leadership training of CQI leadership team at national level and for selected two municipalities;
  • Assist the CQI Leadership Team of two selected municipalities to facilitate at least one facility in the respective municipality to initiate their own CQI process.;
  • Translate all the work process and tools for facilitative supervision into the Tetum language to support Health facilities in the country to establish, implement and sustain their own CQI process; and
  • Support the international consultant to develop a model annual national and municipality level Health Care Quality Improvement Plan and translate these into Tetum to guide investment in supporting CQI processes in Health Facilities. This plan, among others, should include actions to a) Facilitative Supervision of QI leaders and QI teams; b) measure and monitor QI processes in facilities, c) train and support staff involved in the QI process on use of quality tools/techniques; d) communication and advocacy within the sector and outside to support QI process; e) evaluation of activities; and f) recognition and rewarding of successful QI teams.
  • Work with the CQI Leadership Team at MOH and two selected municipalities to follow up on the implementation of CQI plan and help MOH to roll out the Health Care Quality Improvement Plan in other municipalities.
  • Prepare a report in English by documenting lessons learned and key challenges and success and making few action-oriented recommendations.

WORK ASSIGNMENT:

  1. Review and translate the Inception Report developed by international consultant into English to get detail understanding of the scope of work
  2. Work with a lead consult to develop Draft Tetum version of the Continuous Quality Improvement Framework to provide the organizational foundation for Continuous Quality Improvement (CQI) in Public Health Facilities in Timor-Leste through:
  • Organizing and facilitating the consultation meetings with MoH and development partners;
  • Arranging and facilitating field visits to at least two hospitals, two CHCs and two Health posts; and
  • Translating the proposed CQI framework into Tetum

  3. Support the international consultant to develop context-relevant Maternal and Newborn and Child Health Quality Standards and translate these into Tetum

  4. Support the international consultant to develop Maternal, Newborn and Child Health Services Quality Domains and Quality Assessment Tool and translate these into Tetum

  5.  Support the international consultant to develop Tool Book for Quality Improvement of Maternal, Newborn and Child Health Care in Public Health Care facilities in Timor-Leste for use by QI leaders and QI teams and translate it into Tetum

  6.  Support the international consultant to develop CQI leadership Manual (to be used together with the tool-book) by CQI leaders as a resource book and guide and translate it into Tetum

  7.  Coordinate with the CQI cabinet at MOH to present to Council of Directors of MoH and facilitate the discussion of the CQI Framework, draft Maternal and Newborn and Child Health Quality Standards, Quality Domains and Quality Assessment Tool, Tool Book for CQI of Maternal, Newborn and Child Health Care and CQI Leadership Manual and make adjustment as necessary into the Tetum versions

  8.  Translated the proposed Curriculum and Trainers Guide (linked to above-mentioned manual and tools) for conducting Quality Improvement Leadership Orientation into Tetum

  9.  Organize Quality Improvement Leadership Orientation of National QI leaders and Quality Improvement Team Leaders of two selected Health Facilities (Hospital, CHC and Health Post) of two municipalities and support as co-facilitator

  10. Guide/assist the National QI leaders and Quality Improvement Team Leaders of two selected Health Facilities (one Hospital and one CHC) of two municipalities to conduct Quality Assessment and start the CQI Process and follow through to ensure continuity

  11. Support with the CQI Leadership Team at MOH and at least three other selected municipalities (other than initial two) to follow up on the implementation of CQI plan and help MOH to roll out the Health Care Quality Improvement Plan in other municipalities

  12. Prepare and submit the final report of the consultancy along with full set of deliverables described above.

 

DELIVERABLES AND TIME FRAME:

  1. Inception report that shows at least the following (5 days):

  • Broad understanding of the core issues to be addressed through this work;
  • Key steps in the process (how work will be executed); and inception report that shows at least the following:
  • A work plan with time frame and support needed from MoH, Partners, and UNICEF.

  7. Tetum version of the Maternal and Newborn and Child Health Quality Standards, Quality Domains and Quality Assessment Tool, Tool Book for CQI of Maternal, Newborn and Child Health Care and CQI Leadership Manual presented to council directors and document adjusted based on the input (5 days)
  8. Curriculum and Trainers Guide (linked to above-mentioned manual and tools) for conducting Quality Improvement Leadership Orientation translated into Tetum (5 days)
  9. Quality Improvement Leadership Orientation of National QI leaders and Quality Improvement Team Leaders conducted for at least 25 people in total (10 days)

    10.1 Quality Improvement Leadership Orientation of National QI leaders and Quality Improvement Team Leaders of two selected Health Facilities (Hospital, CHC and Health Post) of two municipalities conducted

    10.2 Establishment and training of CQI teams in selected facilities

    10.3 Report of quality assessment and start the CQI Process in two selected Health Facilities (one Hospital and one CHC) is prepared and submitted into Tetum

    Timeframe for deliverables 10: 10 days

     11. Monthly progress report of at least two health facilities (one for each) on CQI implementation status with at least the progress of the following (120 days):

  • Establishment and training of CQI teams in selected facilities,
  • Budgeted Quality Improvement plans of selected facilities and implementation status, and
  • Baseline quality assessment reports and 3 monthly quality improvement progress assessment reports of selected facilities

     12.1 Prepare a report in English by documenting lessons learned and key challenges and success and making few action-oriented recommendations.

     12.2 Finalize Tetum versions of all deliverables as mentioned in #2 to 9 which should be ready for printing.

Estimated Duration of Contract: This consultancy is expected to be started in end of March 2018 to Feb 2019.

Qualifications or Specialized Knowledge/Experience Required:

  • Bachelor in the clinical medical discipline, or nursing or midwifery with a minimum of 5 years of work experience.
  • Proven experience in designing and implementing quality processes in public health care setting in developing countries.
  • Proven experience of training public healthcare managers, and health workers on quality improvement processes and quality leadership.
  • Excellent English and Tetum writing and documentation skills including skills in leveraging MS Words, MS Excel and MS Powerpoint for document organization.
  • Proven track record of working with the Ministry of Health in Timor-Leste.
